Health Questionnaires
All first year students receive a Health Questionnaire, if for any reason you have not, these can be obtained from the Health Centre. All other years are invited to attend the nurse, in the event your health status has changed since completing the Questionnaire.
It is very important that you complete this questionnaire as it ensures we have a baseline record of your health status in the event of an accident or incident, thus enabling us to administer appropriate medical care.
If there are any particular medical conditions/disabilities that may impact on your academic performance, it is important that you inform the nurse or Disability Officer and they can be addressed accordingly, with your permission (see end of questionnaire).
